Individual Authorization |

A card holder, other
than a debtor in a pending bankruptcy proceeding,* can charge court
fees or costs by presenting their card at the Clerk's Office. |

Blanket Authorization Form |
|
|
|
A card holder can submit a blanket
authorization to the Clerk's Office, which will permit any employee
of the firm/company to use the credit/debit card for payment of court
fees and services. |

|

One Time Authorization Form |
|
|
|
This method is normally used when
the card holder is located outside the immediate area. A card
holder who submits the one time authorization form to the Clerk's
Office will be permitted to charge court fees and services. |

* Credit transactions
by a debtor in a pending bankruptcy proceeding will only be accepted
if a debit card
is presented. |
